This form is meant to assist in the Home Visit during the application screening process, and is not meant to be all inclusive. Each visit is different. The questions on this form should be answered but you may add your own questions during the visit. Please note your own observations on the last page under "Additional Visit Comments". Taking photos of the fence and emailing them to the person who asked you to do the visit is extremely helpful. Thank you.
